The district nursing service provides a range of nursing services to people in their own homes in the South Rangitikei district.

District nurses in the Rangitikei district provide a comprehensive range of quality, patient-focused, professional and holistic nursing services that are primarily home based. They provide short-term health services in the community on discharge from hospital or as referred by a health professional:

  • skin and wound care
  • supplies – ostomy dressings or continence appliances
  • assessments for short-term services – home help, Meals on Wheels, personal care
  • access to equipment and aids on short loan – commodes, toilet seats, shower stools
  • monitoring and supervision of patients with complex needs and chronic health problems
  • patient/carer education
  • access to specialist nurse services – palliative, diabetes, respiratory, stoma therapy, continence
  • referrals to other support services – social worker, occupational therapist, mental health team, physiotherapist, Access Ability.


Referral is by GP, hospital or another health professional.
